Open Enrollment 2021-2022:

Key Points

· No enrollment form needed if you are not adding or dropping coverage.

· Medical rates have increased from last year.  We highly encourage employees
to continue to address medical issues as soon as possible to prevent long term
issues and to get wellness visits annually.

· Long-term and Short-term Disability rates will change according to your 2020 W-2 and age.

· Life insurance rates will only change if the employee’s age ends in a 0 or a 5 on 9/1/2021.

· One Call will still be a benefit, but will have a new logo and name (KIS)

· Express Scripts is still correct for all prescriptions . One additional benefit available for specialty meds is called SavOn

You may choose to purchase benefits in increments of $10,000. Employee Life & Spouse Life have a minimum benefit amount of $10,000 with a maximum benefit of $500,000. Child Life (for eligible children 14 days or older) you may choose a minimum benefit amount of $2,500 to a maximum benefit of $10,000.
