· We have switched from Guardian to Mutual of Omaha for Life, Dental, Vision, and Disability insurance.
· Effective September 1, 2019 we will no longer be using TelaDoc.
· All Dental is now under a PPO plan
· We are adding Voluntary Short-Term Disablity coverage
· This 2019-2020 plan year preventive and wellness care visits will have a $0 copay.

Child Life (for eligible children 14 days or older) you may choose a minimum benefit amount of $2,500 to a maximum benefit of $10,000.
